Mandatory Immunization Requirements

For more information, visit www.health.state.mn.us/immunize

Without current immunizations, students will not be able to start school in September. The Minnesota Department of Health has adopted new immunization laws that went into effect in the Fall of 2004. Mabel-Canton students, including incoming kindergartners, 7th graders, and new students to the district will need to meet State requirements or have an exemption on file. Students not meeting these requirements will not be able to attend classes until families provide the required information to the school nurse.

Following are the immunization requirements:

Varicella Vaccine
*Kindergartners and Grade 7 students need to have the Varicella (chicken pox) shot or have documentation on file that they have had chicken pox.

Measles, Mumps, Rubella Vaccine
*Incoming kindergartners and Grade 7 students also will need a second measles-mumps-rubella shot.

Diphtheria, Tetanus Toxoid, and Pertussis Vaccine (DTP)
*Students Grades 7 through 12 are required to have a tetanus-diphtheria booster shot after age 11. Students must also have a record of their baby series of DTP, Polio and Hepatitis B Series.

Shot records or a signed notarized exemption must be on file in the Nurses Office. Please call the school if you have any questions or wish to verify your son/daughter's immunization records.
